Thanks for the feedback on JotForm. It has received more attention than I anticipated. I am not very clear what do you mean for simple database backend. It can already collect the data on our server. Do you think it would be something you would use if it generated some PHP and Mysql code so you can collect the data in your site?

Sorry for being so vague. I'm currently working on a database engine that differs from most in that it's inherently schema-free, and tolerates disparate data easily.

To expose the power of such a database, I need to marry it to a front end that allows people to easily modify the structure of the data that's input and how its displayed. Your web based form builder is good start on exactly that.

What this will achieve is a simple, user oriented development environment. One that lets users build continually evolving applications to meet continually evolving business needs.

There is a new online form builder / workflow application called The Blue Form.
It features a great AJAX style drag-and-drop form builder as well as the ability to assign approval workflows to the forms you create. It is more of a process automation solution aimed at medium sized companies, rather than just an online form builder.
